Geriatrician/AGNP-BC
Summary/Objective
Essential Functions
- Maintains patients well-being and independent functioning by diagnosing and treating illnesses and injuries that commonly occur with aging.
- Provides leadership and expertise for FHS’s quality and clinical effectiveness regarding patients 65 years of age or older.
- Evaluate and optimize the patient care flow throughout the practice including length of patient visit, process to assure appropriateness of medical care, patient satisfaction, patient safety, and ancillary resource utilization.
- Refers patients to other specialists if they suspect more serious health problems.
- Work with other members of the team to coordinate care, including but not limited to our Dental Director, Behavioral Health Providers, Dietician, etc.
- Informing patients and families about advance care planning, including giving up activities, transitioning to supportive living long-term care alternatives, hospice care, etc.
- Work to achieve effective care management processes and develop new initiatives to improve clinical quality.
- Provides for the diagnosis and treatment of health center patients within the scope of his/her license to practice medicine.
- Provides follow-up on patients seen including chart notations, lab review, and referrals to specialists.
- Participates in continuing education and professional development.
Required Education and Experience
- Hold either an M.D. or a D.O. with specialization in geriatrics OR be an Adult-Gerontology Primary Care Nurse Practitioner – Board Certified.
- Must hold a diploma from an accredited school.
- Active license to practice in the State of Ohio.
- Active DEA.
- Successfully completed residency through an accredited program, if applicable.
- Board Certified in Family Medicine or Internal Medicine with a specialization in Geriatrics.

Associate Medical Director
Summary/Objective
Family Health Services is a Federally Qualified Health Care Center located in Sandusky and Norwalk, Ohio and serving the Erie and Huron County area. We are currently looking for an Associate Medical Director. This position would be responsible for ensuring the provision of quality clinical services delivered by the staff of Family Health Services (FHS). The Medical Director has chosen to delegate specific responsibilities of the Medical Director position, aside from physician peer review and discipline to the Associate Medical Director. This position will assume responsibility for the delivery, coordination and supervision of medical services provided at FHS.
Essential Functions
- Routinely meets with Medical Director to review clinical operations.
- Provides information for inclusion in written performance evaluations for all direct report staff and submits written performance evaluations in accordance with policy requirements.
- Identifies the need for staff education and assists in the development of in-service programs for clinical staff members.
- Establishes and maintains clinical supervisory systems which allow staff growth and broaden the scope/efficacy of clinical services provided by staff.
- Provides leadership and counsel to clinical personnel to enhance staff development.
- Supervises the maintenance of medical records assuring accuracy, completeness and compliance with licensing regulations and legal and ethical standards.
- Supervises outgoing referral program.
- Serves as a Lead of the Risk Management Committee and serves on other committees as assigned.
- Participates in the development and oversight of organizational policies and procedures.
- Oversee development and implementation of all requirements for Patient Centered Medical Home (PCMH) certification and Chronic Care Management (CCM).

Quality Director
Summary/Objective
Family Health Services is a Federally Qualified Health Care Center located in Sandusky and Norwalk, Ohio and serving the Erie and Huron County area. We are currently looking for a Quality Director. The Quality Director will be responsible for leading the organizations Quality programs ensuring compliance with HRSA regulations. This position functions as the leader of all quality projects throughout Family Health Services, including, primary care, dental, behavioral health and any future service lines that are added. The Quality Director will build the quality program and oversee the evaluation of programs and services including UDS, Patient Centered Medical Home and other outcomes. The director has the responsibility to ensure the highest quality services are provided to patients throughout the organization. They will plan, lead and direct process and overall quality improvement activities that produce better patient care and more efficient operations.
Essential Functions
- Provide overall direction, coordination and implementation of the quality programs at Family Health Services.
- Implements strategies and plans for quality improvement within service lines in collaboration with Directors and providers including monitoring and presenting internal quality measures, initiatives and plans.
- Oversees CQI audits on patient charts to ensure best practices are being met.
- Coordinates data collection and submission for HRSA required reporting, including the annual UDS report.
- Participate in and assist in preparation for external audits and site visits by accreditation or regulatory agencies including HRSA, 340B, etc.
- Oversee development and implementation of all requirements for Patient Centered Medical Home (PCMH) certification and Chronic Care Management (CCM).
- Monitors FTCA application and compliance.
- Develops reporting through eCW/Azara to accurately pull data to analyze for PDSA studies and as indicated for operations, grants, etc.

Outpatient Clinical Dietitian PRN
Summary/Objective
The dietitian will participate as a member of the interdisciplinary healthcare team to facilitate the continuum of care and education of patients with nutrition-related chronic health conditions. Responsible for conducting individual and occasional group patient nutrition education at Family Health Services. Meets with patients and patients’ support members to assess nutrition-related knowledge, self-care habits, and nutritional status. Provides Medical Nutrition Therapy for individuals with various chronic diseases like diabetes, obesity, heart disease, etc. Consults with nurses, physicians and others as necessary to resolve problems with patient’s overall care. Additional outreach education duties at community sites may be required upon request.
The dietitian is accountable for setting priorities, meeting deadlines, and promoting the achievement of patient outcomes. In addition to working with the healthcare team, the dietitian will interact with families and patients in a positive and empathetic manner. Utilizing a team-based approach, the dietitian provides highly proficient care, utilizing evidence-based guidelines, which is patient-centered and supportive of diversity and cultural factors. This position will assist the Food Is Medicine Program director as time permits with other patient, community, and staff education programs (individual or group) as needed and is an active participant in improving the health and well-being of our community.
Essential Functions
- Assesses nutritional status of patients based on input from referring physician, medical records, lab results, blood glucose monitoring trends, various clinical assessment methodologies, and patient consultation.
- Assesses nutrition knowledge and self-care behaviors of patients and their support members. Evaluates patients’ skills in self-care, ability/readiness to learn, and resource utilization. Collaborates with other members of the interdisciplinary team in gathering data relevant to the patient’s health status.
- Collaborates with the patient and the interdisciplinary health care team in developing a patient-centered plan of care. Examines the various treatment options available for implementing the plan of care.
- Designs individualized care plans that are congruent with the preferences, needs, age, educational level, culture, lifestyle, financial resources, physiological status, psychosocial readiness and desired clinical outcomes of the patient.
- Provides Medical Nutrition Therapy and educates clients and support members on the content and strategies to operationalize nutrition goals and assimilate them into long-term behavior changes. Mentors patients and support members in the development and application of clinical problem-solving skills.

Knowledge of dietetic and nutritional needs and can develop clear care plans. Preferred experience in working in a Federally Qualified Health Center. Preferred certification registration with American Dietetic Association.
Expected Work Hours
This position has a part-time, non-exempt, flexible schedule of 15-25 hours per week with the opportunity for full-time employment in the future. Reports to Food is Medicine Program Director.
Travel
Minimal, with possible occasional travel to other site locations within the local area.
Work Environment
This job operates in a healthcare setting. This role requires regular walking to various locations around the building. This role also routinely comes into contact with patients who may have contagious illnesses.
Physical Demands
While performing the duties of this job, the employee is regularly required to talk and hear. This position is moderately active and requires standing, walking, bending, kneeling, and stooping all day. The employee must frequently lift and/or move items over 20 pounds.

Psychiatric Mental Health Nurse Practitioner
Summary/Objective

The Psychiatric Mental Health Nurse Practitioner will provide advanced nursing care for patients with psychiatric disorders and may provide psychotherapy under the direction of a psychiatrist.
Duties/Responsibilities:
- Diagnoses psychiatric disorders and mental health conditions. Distinguishes between physiologically and psychologically based disorders and makes appropriate diagnoses.
- Assesses patients’ mental and physical status based on the presenting symptoms and complaints. Conducts individual, group, and family psychotherapy for those with chronic or acute mental disorders.
- Documents patients’ medical and psychological histories, physical assessment results, diagnoses, treatment plans, prescriptions, and outcomes. Writes prescriptions for psychotropic medications as allowed by state regulations and collaborative practice agreements. Monitors patients’ medication usage and results.
- Evaluates patients’ behavior to formulate diagnoses or assess treatments.
- Educates patients and family members about mental health and medical conditions, preventive health measures, medications, and treatment plans.
- Collaborates with interdisciplinary team members including psychiatrists, psychologists, and nursing staff, to develop, implement, and evaluate treatment plans.
Required Skills/Abilities:
- Ability to diagnose and treat simple and complex psychiatric and mental health problems from adjustment disorders to serious mental illnesses such as schizophrenia, bipolar illness, major depression, and anxiety disorders.
- Strong skills to manage, coordinate, and evaluate the care and education of the patient and family along the health care process.
- Ability to manage patients who need to be closely monitored.
- Trained to adhere to medications and therapies with complex protocols.

The Dental Director has chosen to delegate specific responsibilities of the position, aside from dental physician peer review and discipline to the Dentist. FHS is a National Health Service Corps (NHSC) approved health care site. This allows the dental provider (DDS/DMD) to be eligible to apply for the NHSC Loan Repayment Program with an award amount up to $50,000 for Full-Time status employees and a 2-year Service Commitment.
Essential Functions
- Routinely meetings with Dental Director to review dental operations.
- Assists the Dental Director in carrying out the dental care plan.
- Meet with patients to discuss and treat dental concerns, performing regular cleanings and other preventative procedures, and establish a plan for better dental hygiene.
- Performing dental procedures, such as extractions, root canals, and filling cavities.
- Correcting bite issues and overcrowding.
- Applying helpful agents to teeth, such as sealants or whiteners.
- Prescribing medications for dental problems, such as pain medications or antibiotics.
- Giving clients sedatives or anesthesia prior to administering treatments.
- Ordering diagnostic measures, such as x-rays, models, etc.
- Using tools, such as drills, probes, brushes, or mirrors, to examine and treat teeth and mouth.
- Keeping records relating to the oral health of patients and the treatments given to them.
- Managing and communicating with other staff members to provide care to patients.

Required Education/Experience

Dental Hygienist
Summary/Objective
The Dental Hygienist is responsible for the direct delivery of oral hygiene and appropriate preventive services to dental patients of the center. The Dental Hygienist will also be responsible for providing appropriate oral health information to FHS participants and staff, including outreach events.
Essential Duties & Responsibilities:
- Assists in the oral health management of all clinic patients.
- Obtains baseline oral hygiene assessment data based upon accepted principles of oral hygiene and periodontal care.
- Records patient-hygienist transactions as they occur in the patient’s dental record so that the dental record accurately and completely reflects the nature of the contact, the condition of the patient and the care or treatment provided.
- Provides oral health education and appropriate individual counseling for all center dental patients.
- Provides clinical oral hygiene services including dental prophylaxis, scaling, sealant application and fluoride application consistent with accepted professional practices and standards and in compliance with applicable state law and the center’s clinical protocols. Works with Dentist to assist in coordination of care and referral for patients receiving preventive services.
- Performs independent procedures as delegated and directed by a dentist in accordance with state regulations and law as well as Oral Health Department protocol and directive.
- Assists in the maintenance of all Oral Health Department areas in compliance with department directives and policies as well as center policies and procedures relative to infection control, exposure control and safety issues.
- Receives and places necessary telephone calls consistent with professional matters, clinic business and patient care of the Oral Health Department.
- Assists with various clinical and administrative functions of the center as appropriate and time permits.
- Ensures the proper disposal of all contaminated or potentially contaminated materials in accordance with Oral Health Department directive and center policy as well as state federal regulation(s).
- Complies in full with the center’s Exposure Control Plan.
- Travels when necessary to meet operational needs.
- As directed by a supervisor, performs other related and/or necessary tasks to achieve organizational and programmatic goals and objectives.
- Responsible for personal compliance with all applicable federal, state, local and center rules, regulations, protocols and procedures governing the practice of dental hygiene and the clinical provision of dental hygiene services as well as those relating to, but not limited to, personnel issues, work place safety, public health and confidentiality.

- Completes behavioral health diagnostic assessments.

Summary/Objective
The Medical Assistant performs duties essential to the patient visit and other duties as assigned.
Essential Functions

Summary/Objective
To support the operation of the clinic by maintaining the front office and representing the office with professionalism. This position requires the ability to multi-task and prioritize responsibilities, handling patient requests, placing outbound calls to patients, and handling all administrative tasks with a positive attitude.
